7.3 Case Study Analysis The Crash of American Airlines Flight 587 – Pilot Training and CRM Failures Overview Two months after two hijacked airliners were flown into the World Trade Center buildings in New York, an American Airlines Airbus A300 crashed in Queens shortly after takeoff from JFK airport. All 260 passengers and crew, plus five people on the ground were killed. Though much of the early focus in the media was on the possibility that this was another terror attack, the NTSB investigation revealed that excessive rudder inputs by the First Officer when encountering wake turbulence led to the separation of the vertical stabilizer and an unrecoverable situation. Furthermore, the NTSB found that the training and simulation programs of American Airlines contributed to the FO’s perception that such large rudder inputs were required to maintain control during wake turbulence encounters.
