Social Element of Personal Wellness

Part 1: Initial Wellness Goal (5%): Choose one element of your personal wellness that you would like to improve in (Social Element), can be from any of the dimensions of wellness dictated below Create a goal for facilitating this behavior change. Your goal can be submitted in written format (double spaced), audio or video recording. It is recommended to be in SMART format but at minimum should include; a brief description of the dimension you would like to improve, an overview of the plan you will implement to reach this goal, and a description of how you will measure your progress.
