For this discussion board assignment, you will analyze the relationship between social determinants of health (SDOH) and community health outcomes using the County Health Rankings framework. This assignment will help you develop a deeper understanding of how social and economic factors influence health disparities in communities across the United States. Part 1: County Selection and Analysis (Initial Post) 1. Visit the County Health Rankings website (www.countyhealthrankings.org). 2. Select two counties within your state start with the county in which you reside. Then, depending on how your county performs, select either a county with lower or higher health rankings. Make sure you have contrasting performance in the counties you select. 3. For each county, identify and analyze: o Overall health outcomes ranking o Three key social determinants of health affecting the county (e.g., education, income, housing, transportation, social support) o At least two specific health metrics that show significant disparities between the counties 4. In your initial post, address the following: o Compare and contrast the two counties based on their health rankings and identified social determinants o Discuss how the selected social determinants might be contributing to the health outcomes in each county o Propose one evidence-based nursing intervention that could address a key social determinant identified in the lower-ranked county o Include at least two scholarly references to support your analysis and proposed intervention 5. In your two peer responses compare the social determinants they identified with those in your selected counties. Suggest an additional evidence-based nursing intervention that could compliment their proposed interventions.
