1. 5 SOAP NOTES EACH ONE DIFFERENT CASE WITH DIFFERENT PATIENT AND MEDICAL HISTORY , EXACT EXAMPLE WILL BE UPLOADED THE INFORMATION NEEDED IS DOWN BELOW

SOAP NOTE TEMPLATES ARE ALSO PROVIDED TO BE COMPLETED

1. Patient encounter documentation is worth 25 points each for a total of 150 points. 1 consult note; 2 H&Ps; 2 Progress Notes, 1 Consultation, & 1 Discharge Summary; 1-Procedure note (optional if applicable); Each student is required to complete six SOAP notes as noted above. The purpose of this assignment is for students to practice writing patient documentation. The procedure note is optional if applicable. One patient encounter documentation must include an explanation related to cost and reimbursement. (Facilitation of learning & Response to diversity; Systems thinking & Clinical inquiry)
