His wife is also present but seems unsure how to support him at home. 2. You are caring for a 72-year-old female patient who was admitted for pneumonia and is being discharged tomorrow. During your shift, you notice she appears weak and struggles to get out of bed without assistance. Her daughter, who lives an hour away, expresses concern about her mothers ability to care for herself at home. The patient also mentions shes having trouble affording her medications. Your assignment is to identify one in-hospital referral you would make for this patient, and one community referral you would make for this patient. Use resources from your own community. Remember to use specific examples. Do not give generalities such as I would refer them to physical therapy. Tell me WHY you would do this, and identify a resource in your community.
