Case Vignette: The Ramirez Family

The Ramirez family presents for therapy due to concerns about 15-year-old Sofia, who has recently refused to attend school. Over the past three months, Sofia has complained of stomachaches in the mornings and becomes tearful or irritable when pressured to go to school. She has missed 18 days this semester.

Family Members

  1. Elena (Mother, 44): Describes herself as very close to Sofia. She checks in with her frequently throughout the day via text and often allows Sofia to stay home when she appears distressed. Elena reports experiencing anxiety when she was younger and says she doesnt want Sofia to suffer like I did.
  2. Carlos (Father, 47): Works long hours. He reports frustration with the situation and believes Sofia is avoiding responsibility. He thinks Elena is too soft and that Sofia needs firmer discipline. Arguments between Carlos and Elena about parenting have increased.
  3. Mateo (Son, 12): Quiet during sessions. When asked about the school refusal, he shrugs and says, Its always about Sofia anyway. His grades have recently dropped.
  4. Sofia (daughter, 15): Reports feeling overwhelmed at school but struggles to identify a clear reason. Denies bullying. Becomes tearful when discussing school. Often withdraws when her parents argue about her attendance.

Presenting Concerns

Sofia reports feeling overwhelmed at school but cannot clearly explain why. She denies bullying. When Sofia stays home, Elena rearranges her work schedule to be with her. Carlos often withdraws from the conflict, spending more time in the garage or at work. Parental disagreements escalate the night before school days. Family dinners have become tense; discussions often center on Sofias attendance. The extended family (grandmother) tells Elena she must protect Sofia from stress.

Observed Interaction in Session

When Sofia becomes tearful while describing school, Elena moves closer and puts her arm around her. Carlos leans back, crosses his arms, and looks away. If Carlos challenges Sofia (Youre fine. Just go.), Elena quickly intervenes (Shes trying!). Sofia then becomes quieter and looks down.

When the therapist asks Mateo how this affects him, Elena answers for him.
