Patient log/ Encounter Format/ Time Logs
Need to create 11 patient log/encounters focused on psychotherapy . Possible diagnoses may include anxiety disorders, depressive disorders, substance use disorders, ADHD, and trauma related disorders. Some patients should have more than one diagnosis. The diagnosis should be common and patient age should range from 17 years old to 80 years old. Below should be the format used for each patient. No specific word count needed as long as all questions answered. Note that this course is focused on psychotherapy and should be the primary focus and not medication management or other nonrelated treatment that does not meet the course requirements. Average therapeutic techniques should be utilized. Diagnoses may be repeated.
