Assignment Overview:

This week, you will explore how social determinants of health (SDOH) affect individuals’ ability to access and benefit from healthcare services. You will analyze the life circumstances of a fictional or de-identified real person from an underserved population and apply your understanding of SDOH, population health, and the nurses role in promoting equity.

This case-based assignment gives you the opportunity to develop critical thinking and advocacy skills through a population health lens.

Instructions:

Step 1: Select or Create a Patient Persona

Choose one of the following underserved patient populations (or a similar population approved by your instructor):

  • A member of a marginalized racial, ethnic, or LGBTQ+ community

Create a brief narrative of their daily life, focusing on challenges related to housing, food access, employment, safety, healthcare, and social support.

Step 2: Analyze the Impact of SDOH

In a 23 page paper (not including title and reference pages), address the following:

  1. Describe the patient persona and their background (no real names or PHI).
  2. Identify and explain at least three social determinants of health that impact their well-being.
  3. Discuss how these determinants affect their access to care and health outcomes.
  4. Propose at least two nursing interventions or community resources that could help improve their situation.
  5. Include a reflection on how this case study informs your future role as a masters-prepared nurse in promoting population health and equity.

Step 3: Format and Submit

  • APA 7 formatting (title page, in-text citations, reference page, double-spacing)
  • Minimum of two scholarly sources, including at least one from course materials
  • Submit as a Word document or PDF in Canvas
