Provide a plan to collect data for your QI project. This is a component of the “how” of your plan.

You must articulate a clear plan for data collection. Consider the following key elements:

Data Selection: Specifically, what data points will you track?

Methodology: What approaches or tools will you use to gather this information?

Roles & Responsibilities: Who will be responsible for collecting data or conducting rounds? How frequently will this occur?

Outcome Measurement: How will you quantify success?

Visualization: How will you present the results visually (e.g., run charts)?

Hypothesis: What are your expected outcomes?
