Data was collected to identify specific cases of people diagnosed with a chronic disease. Your organization is interested in knowing where the population is most affected so resources can be moved to areas that need them.

Your department is continuing to work on the strategic planning for a nationwide health care delivery organization. Youve been asked to make a recommendation on the optimal locations to allocate resources for the best return on your investment.

Write at least a 700-word analysis report based on the prevalence rate data. In your report:

• Create a bar graph in Excel to identify the cities with the highest counts of chronic disease cases. Include this graph in line with your report.
• Identify the top 5 cities for chronic disease cases based on your graph.
• Identify how many chronic disease cases each of those cities has based on your graph.
• Calculate the prevalence rate per 100,000 people within the Prevalence Rate spreadsheet.
• Create a bar graph in Excel to identify the cities with the prevalence rate. Include in line with your report.
• Identify the top 5 cities for the prevalence rate based on your graph.
• Analyze what both graphs, case count, and the prevalence rate tell you about the chronic disease cases.
