Interview, Plan of Care, and Educational Material

Instructions:

1. Select activity A or B. You may use a friend or a family member. Disguise all identifying information to maintain privacy.

Activity A: Interview an older adult who has a chronic cardiovascular disease (e. g., hypertension, peripheral vascular disease, heart failure, coronary artery disease).

Activity B: Interview an older adult or the caregiver of an older adult dealing with a neurodegenerative disorder (e. g., Parkinsons disease, Alzheimers disease).

2. Address the following:

• Describe the chronic condition, client risk factors, and client manifestations.
• Explain whether the condition limits the client from performing normal activities.
• Identify the support that the client requires to manage the chronic condition and maintain normal activities (e.g., family, friends, community resources, healthcare providers).
• Develop a plan of care for this client that includes diet, exercise, medications, home equipment, and interdisciplinary collaboration.
• Create an educational material for the client’s chronic condition and include health promotion and illness prevention strategies. Keep in mind the health literacy of the client.
• Use the APA format to construct your paper.
