Please choose a health disparity issue and write about it following these guidelines:

1. Title Page

• Format according to APA7 guidelines.

2. Table of Contents

• Create a dynamic table of contents in Word, which will be updated as the project evolves.

3. Section Headers and Content

Your paper should include the following sections:

a) Introduction/Literature Review (2-4 pages):

• Explain the significance of your chosen issue and summarize existing knowledge on the topic.
• Include a brief literature review using at least six (6) evidence-based resources.

b) Social Determinants of Health (SDOH) or Socio-Ecological Model (SEM) (1 page):

• Identify and analyze at least two SDOH/SEM factors contributing to the issue.
• SDOH/SEM factors are selected with an explanation of their relationship to health issues

4. References

Checklist:

• Title, Abstract, TOC, Headings and Subheadings
• Literature Review
• Data Resources – Each data sources list at a minimum: years of data, type of data (survey, EMR, lab values, etc.), population, questions related to the topic.
• 6 peer-reviewed articles, 5 data sources for the Data Resources section
• Citations/in-text citations are correctly formatted in APA7
• SWOT Analysis

– Attempted SWOT analysis

• Recommendations

– Recommend how to improve data and how to address the issue.
