SOAP Note PowerPoint Recommendations Submit PowerPoint presentation using approximately 20 slides. Slide 1 – Title Page that lists: the primary risk factor of the patient. Slide 2 Data that includes: Past Health Biographic Data Family History Review of Systems Functional Assessment Perception of Health Slide 3 – Objective Findings (usually 2-3 slides) that includes: Measurement data General Survey Review of pertinent abnormal findings from the physical examination Slide 4 – Analysis of Findings (usually 1-2 slides) that includes: all actual and potential health problems for the patient based on the subjective and objective assessment findings. actual and potential health problems are pertinent and relevant. Slide 5 A plan for Health Promotion (usually 4-6 slides) that includes: an analysis of findings. relevant age-specific and cultural perspectives. appropriate members of the health care team for inter-professional collaboration with a rationale to support why they were chosen. two peer-reviewed nursing journal references to support your plan. a summary of your subjective and objective examination findings an analysis of the examination. a plan for health promotion using a SOAP note format. Reference Slide-references in APA format Read Gagan’s (2009) article and view the Soap Notes video to familiarize yourself with the components of a SOAP note before developing your presentation. Some suggestions for creating your PowerPoint Slides should be clear and to the point. Avoid loading too much information on one slide. Label slides – Subjective or Objective, etc. Write information in a bulleted format. Avoid writing in paragraphs. Follow the rubric instructions closely. For example, age-specific and cultural perspectives should be incorporated into the health promotion plan. The instructions call for a review of pertinent Abnormal Findings from the physical exam only. Provide a minimum of 2 peer-reviewed nursing journal references that support your plan. Don’t forget your reference slide at the end of the presentation! Incorporate long-range plans for health promotion. Not just the acute phase of an illness. Incorporate color, graphics, and images.
