Purpose: This assignment allows you to apply what you have learned about the neurology of swallowing to two specific patient cases. This will help you to think critically about the information you have mastered, gain an appreciation for how it might present clinically, and analyze the effects of neurological impairment on a patient or clients daily life. You will exhibit your knowledge and case study analyses via a thorough and visually pleasing prezi presentation.
Instructions: Imagine that you are a medical SLP that works at a 500 bed hospital that is a stroke center and trauma center. Every day, you use your critical thinking skills to make the best, safest recommendations for your patients, always relying on evidence based practice. For this assignment, create a prezi presentation that presents the two patients and exhibits your evaluation plans, answering all questions below. Your response should show evidence of critical thinking skills, informed by course lectures and readings.
Submission should be a
with at least 10 slides or screens that answers the questions below.
- Patient One – You received a swallow evaluation order for a female 67 year old patient in the ICU. The chart review reveals an acute brainstem CVA.
- What further information might you look for in your chart review? List at least 3 more areas of information that you need to know before seeing this patient.
- What components will you include in your evaluation?
- Which phases of swallow do you predict deficits (considering the site of lesion)?
- What recommendations might you make for this patient (considering the site of lesion)?
- How would you describe the risk of dysphagia and aspiration for a patient with a brainstem CVA?
- How might this swallow deficit impact this individual and their overall quality of life?
- Patient Two – You receive a swallow evaluation order for a 28 year old male patient who is diagnosed with a traumatic brain injury and respiratory failure after experiencing a motor vehicle crash 10 days ago (resulting in his emergent hospital admission). He self-extubated last night after a 9 day intubation. The nurse reports he is alert, pleasantly confused, and impulsive. The nurse also explains his voice is weak, quiet, and whispered.
- What concern might you have considering the nurses description of the patient’s vocal function? (Explain the connection between vocal fold and swallow).
- What swallow deficits might you predict, specifically based on the patient description?
- Lets say you end up recommending an instrumental study. Which instrumental study might reveal the most information, considering his intubation and vocal function? Explain why you selected the specific instrumental study.
- How might this swallow deficit impact this individual and their overall quality of life?
