For this assignment you will create a NARRATED powerpoint presentation on the case scenario below in a SOAP note format. This assignment is a little different whereas we will give you the diagnosis for the patient and you will then discuss the following key items related to the provided diagnosis. Be sure to address ALL key items using a powerpoint presentation. You will also provide recorded narration for each slide (you will do this within the tool bar in powerpoint). Case Scenario: A 6 mos old male presents to your clinic with his mother. Mom states he has been sick for several days with runny nose, congestion, fever, and cough. Mother states he has been breathing fast and his cough sounds like he is “barking”. He is also not sleeping well due to the cough. He does attend daycare and has an older sister in preschool. The infant has no past medical history, was born full term without any complications, and his immunizations are up to date. He does not have any known allergies and does not take any routine medications. There is family history of asthma on paternal side. His vitals in the clinic are T 100 P 120 RR 54. Weight is 17 lbs. After examining the patient, you diagnose him with Croup. Diagnosis: Croup Directions: 1. Create a narrated powerpoint presentation addressing the following key points from the provided case scenario. The best way to record narration is using the “record” feature located in the toolbar within the powerpoint application. Use these bulleted key points below as the headings for your slides. Provide a separate slide for each bulleted item below. CC HPI Allergies Medications PMH FMH Sochx ROS (describe the pertinent positives and negatives that would be reported to you with diagnosis provided) Vitals PE (describe positive and negative exam findings you would expect to see with diagnosis provided) Diagnostic tests (this includes any labs/tests done during the visit to help lead you to your diagnosis) Risk Factors (this includes the risk factors for the diagnosis provided) Primary Diagnosis (Provide a paragraph explaining definition of primary diagnosis, pertinent positives and negatives for each diagnosis, and ICD10 code. Also discuss pathophysiology of the provided diagnosis. Support with evidenced based literature). Differential Diagnoses (Provide TWO differential diagnoses. For each differential diagnosis, provide a paragraph explaining definition of differential diagnosis, pertinent positives and negatives for each diagnosis, and ICD10 code. Support with evidenced based literature). National Clinical Guidelines (State the national clinical guideline you used in APA format and then provide the LINK to the national clinical guideline you used to develop your treatment/plan section on your actual slide). Plan – include the following. Include ALL items below. If an item does not apply, then state N/A.
