Community Health Improvement Plan (CHIP) PowerPoint Presentation (GLOBAL) A Community Health Improvement Plan (CHIP) is a strategic framework developed to address a specific health issue within a community. It involves a comprehensive assessment of the community’s needs, identification of priority health concerns, and the development and implementation of evidence-based interventions to improve the health outcomes of the target population. Building on the CHIP project completed in NUR 415, this project will have a global approach. You will work in groups that will be assigned by the instructor. The group will choose a country or global community of interest. The community must be approved by the instructor. You will need to collect data related to the community just as you did in your data collection last semester. Remember, this is a larger community. When you are looking at your Intervention and Plan, it needs to be something that can be implemented on a large scale. You are no longer looking at the individual or even one city or town. You are looking at the larger population. The presentation should include at least 10 slides (including the title and references slides). The presentation should range between 5-10 minutes. Every student in the group must participate in the presentation. Do not just read from the slides. Give us more details. Use a mix of paragraphs, bullet points, and pictures on the slides. You will also complete peer evaluations for this project that will considered during the grading process. Key Components of the Project: 1. Community Profile: o For the profile you will need general population statistics, characteristics, and groups within the community. Include ethnic statistics, religions of the community, popular foods and type of medicine available (government run, private insurance, socialized medicine, etc.). 2. Societal Factors/Health Disparities o Identify community health needs and disparities, existing resources, and a general analysis of the physical and social environment. 3. Priority Health Needs: o Identify the most pressing health problems based on their impact on the community’s health. o Prioritize one health need: Describe why this is a priority need for the community and how it impacts the community as a whole. 4. Intervention: o Describe an evidence-based intervention that addresses the priority health need. o Describe the impact this intervention could have on the health of this community. 5. Plan: o Create a plan for implementing the intervention. o Identify important partnerships with community organizations and stakeholders. (Do you need the CDC, Health and Human Services, or Ministry of Health involved?) o Include actions and individuals needed for implementation. Scholarly Writing Use clear and concise language Avoid jargon and overly complex sentences. Proofread carefully Check for grammar, spelling, and punctuation errors. Presentation Requirements Required APA format for citations. At least three scholarly peer-reviewed sources from the last 5 years are required. A minimum of six resources total must be included. Cite all sources used in your presentation, including direct quotes, paraphrased information, and ideas. The presentation should include at least 10 slides (including the title and reference pages). The class presentation should range between 5-10 minutes. Ensure that the slides are easily readable.
