CJP

You will be using the blank CJP for this assignment, it will open once you open it in a word document. It will look, staggered at first until it is open under a word document.

you can refer to CJP example for reference and guidance.

I have attached a form called Key point mandatory reminders, That gives guidance on answering each section and mandatory directions that we need to follow. For example, some areas need citations and some areas only need a total of three sentences. Others may need a citation. You can find the answers on this form.

I have attached a document called NPW for CJP. You can use this document which has the patients information for the blank CJP. It will give you information on this patient to be able to answer the questions on the blank CJP file. If we dont have all the answers on the NPW for CJP form, like ethnicity or social determinants or little things that The CJP is asking for, You can make something up as long as the questions are not left blank.

There is another document called CJP medications. These are the five medications that we could use to type them into the CJP blank document form.

At the end of completing the assignment we can refer to the CJP rubric file, which I have attached as well to make sure we did not miss any requirements. This rubric will be used for grading.
