Caring for clients with chronic disease

Students are to read Chapter 16 of Foundations for community health workers (3rd ed.). and explain the different types of models on pp.453-458. Describe how these models vary and may impact the care you give your client with a chronic health condition. Choose one of the models you find most favorable for your clients’ health and explain why this model was chosen. This assignment must have a minimum of 500 words with two scholarly references in APA format.
