My Topic for this project is – Improving medication adherence through nurse-led education and a tool.

Project Outline (25 Points)

• What is the Issue You Are Addressing? (1-2 sentences)
• Clearly define the problem or healthcare issue your project is addressing.
• Example: This project addresses the increasing prevalence of hypertension among adults aged 40-60 and the lack of proper tracking and education on managing blood pressure.
• Be specific about the healthcare problem and why it matters.
• Background and Prevalence of the Problem (1-2 paragraphs)
• Provide a brief overview of the healthcare issue, including:
• Prevalence: Use statistics to describe the scope of the problem (e.g., “In the U.S., 1 in 3 adults are affected by hypertension.”).
• Consequences: Explain the potential negative health outcomes associated with the problem (e.g., “If left untreated, hypertension can lead to stroke, heart failure, and kidney disease.”).
• Current Gaps: Briefly discuss any existing challenges in addressing the issue (e.g., “Many adults are unaware of their hypertension or lack the resources to track their blood pressure regularly.”).
• Who Is the Target Audience of Your Project?
• Define who your project is aimed at, including:
• Age group: (e.g., adults aged 30-50, elderly population).
