After reading Choosing Strategies for Change, write a 500-750-word analysis that applies the stages of change to your proposed project.

Part 2: Logic Model Creation

Review the topic Resource titled “Logic Model Terms and Examples.”

Then, select one of the templates from the topic resource titled “19 Fillable Logic Model Templates and Examples” to create a logic model to start planning your project proposal (helping children in low income communities with mental health issues).

Your logic model should use two to three scholarly sources to support your ideas and include:

• Inputs
• Outputs (tangible results of activities, including the role of assessment, diagnosis, and treatment when working with behavioral health clients)
• Short-Term Outcomes
• Long-Term Outcomes
