Transition to Implementation (approx. 1-1.5 pages) Using Healthcare Strategic Planning by John Harris and Meredith Inniger, 5* edition, Phase 4 (Transition to Implementation): Discuss three to four of the following for your initiative: Leadership/oversight structure Stakeholder engagement Communication and culture Resources (people, money, technology) Change management/timing Identify one major barner to successful implementation and propose one concrete mitigation strategy, drawing on ideas from the textbook. Explain how you would monitor progress, including: a few key metrics, and how often and by whom progress would be reviewed. 3. 12-18-Month Implementation Snapshot (approx. 1-1.5 pages) Outline a high-level implementation plan for the first 12-18 months. Include at least 5 major actions; for each, state: the action, the responsible role (not a person), approximate timing (e.g., Months 1-3), and one success indicator. Briefly describe who the key stakeholders are and how you would keep them informed during this period. Conclude with one insight you gained from applying the textbook’s framework to a real initiative and how it affects your thinking as a healthcare leader.
