General Instructions

Download the and complete the required information using the template.

Include the following sections (detailed criteria listed below and in the grading rubric)

1. For each medication listed, complete the following:
2. Include indication, approved ages, target symptoms, and affected neurotransmitters.
3. For each medication, include all available formulations (if IR, XR, ODT, LA), duration of action, and peak (if noted).
4. List notable side effects.
5. List client education instructions for each formulation of each medication.
