Identify a screening tool and briefly describe its purpose and target population. For example, the PHQ-9 screens for depression screening, and the Modified Centor Criteria determines if rapid strep testing is indicated. The tool identified must be unique. If another student has already identified the tool, then you must pick another tool. Do not select tools related to DEIB, SDOH, or cultural competence because those tools were detailed in the Week 3 discussion. Share a specific example of how this tool has improved your practice or patient outcomes in one of the following ways: enhanced assessment accuracy, streamlined workflow, improved patient education and engagement, and assistance with better decision-making.
